Putative Father Registry Search
The Montana Office of Vital Records maintains the Putative Father Registry. This confidential registry allows a man who believes he may be the father of a child to protect his parental rights and ensure he is notified of any legal proceedings regarding the child, such as a termination of parental rights or an adoption.
How to Register (For Potential Fathers)
If you believe you may have fathered a child and wish to receive legal notice of court proceedings, you must add your name to the registry.
- Deadline: Your registration must be received no later than 72 hours after the birth of the child to ensure your right to legal notice.

Important Notes
- Child Support: Information provided to the registry may be used for the purpose of establishing child support obligations.
- Update Your Info: If you have already registered and your address changes, it is your responsibility to notify our office in writing to ensure you receive any future legal notices.
How to Request a Search (For Agencies & Legal Parties)
To search the registry for a potential father, please mail a complete "Search Packet" to our office.
Your Search Packet must include:
- A Written Request on Letterhead: State the child’s name, date and place of birth, and the mother’s full maiden name (including her date and place of birth).
- Legal Consent: Include a notarized consent form from the birth mother giving permission for the search. If the mother is unavailable, a Court Order is required.
- Search Form: Complete the upper portion of the
